Community Assisted Transport Service – Pine Rivers
CATS are a service provider, which is funded by Home and Community Care (HACC) and delivered by the Pine Rivers Shire Council. The service is funded to provide door to door transport and some social support to frail aged and/or a person with a moderate to profound disability, within the guidelines for service to those who are unable to access public transport due to health and accessibility reasons. Clients must be HACC eligible, to receive this service. Eligibility is determined by an On-going Needs Assessment (ONI) and service assessment that is conducted by the Co-ordinator at a home visit, or over the phone. Some of the transports provided by the service include transport to medical appointments, allied health appointments, shopping, and some social events. CATS are unable to provide: * Education or employment transports * Inter hospital transports including discharges from hospital * Transport after a general anaesthetic or medical procedure unless a carer is present * Transport to people living in hostels, supported accommodation, community or group homes, or nursing homes * Transport where the person is unable to transfer from wheelchair to vehicle independently, or with basic assistance from driver, unless travelling in wheelchair assisted vehicle * Have support need which are above the ability of the volunteer driver to support. CATS office hours are between 8:30am – 12 noon and 12:30pm – 4:00pm. Due to the high demand for the service and limited funding, please be aware that the service may not be able to meet your needs in the immediate future.
